Growing Well Cincinnati

Health Match is an online process that matches interested health care providers with individual CPS schools who are seeking health partners to serve their students in school. Access to health services is not available for many school aged children in Cincinnati. Growing Well Cincinnati’s matching process hopes to improve access to health services for CPS students. This process is for both school based health services and school linked health services.

Parameters for Partnerships for GWC:

Serve with fidelity
Be culturally compatible
Be a true CLC partner (serve as part of CLC committee for school, communication with school and other partners, +/- become an LSDMC member)
Agree to agree
Maintaining partner standards, impact.
Location: Co-Locate in a CLC or school link?
Service hours (plan/provide after school, weekend, and summer access to services in order to meet definition of a medical home)
Determine what school community wants in terms of determined need.
