

Planned Pilots for School Health Model
- Development of health services model which could be duplicated in other schools
- Business plan for sustainable, integrated services
- Pilot selection was based on criteria list developed by Collaborative
- Key component is request by school, community, LSDMC for health partner through the community engagement process
- Oyler school in Lower Price Hill is the first pilot site
Key Elements of Collaborative
- Financial Plan needs to be a single model
- Funding options: Fee for Service Billing, Medicaid, Sliding Scale Private Pay, Grants, Public Funding
- Billing by Collaborative and Payment to Providers based on Agreements
- Service Models at individual schools will vary according to school and providers, not a “one size fits all” model
- Transportation options will be key for offsite services
- Single forms for consent, data access, billing planned for use across all school sites
- Connected data base for billing and medical information
Outcome Measurement Tools & Evaluation
- Baseline data collection in development
- School Health Index: CDC evaluation tool to be used for pilot schools
- Outcome Measurements will include health indicators and community/school indicators and be combined with other School Partners into a single data set
